This violation was observed on or about June 12, 2012, on the exterior of a newly built house where employees were exposed to an approximately 14 foot fall hazard while performing siding work on pump jack scaffolding that rested on the soil and did not have base plates or mud sills.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  The employer does not protect employees on a scaffold from falling when working more than ten feet above lower level.  This violation was observed on or about June 12, 2012, on the exterior of a newly built house where;  a) an employee on the east side of the structure was exposed to an approximately 14 foot fall hazard while performing siding work on pump jack scaffolding that did not have a guardrail or personal fall arrest system.   b) an employee on the south side of the structure was exposed to an approximately 19 foot fall hazard while performing cornice work on an elevated wood platform  that did not have a guardrail or personal fall arrest system.  c) an employee on the west side of the structure was exposed to an approximately 15 foot fall hazard while performing siding work on an elevated wood platform  that did not have a guardrail or personal fall arrest system.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories shall be fabricated from metal plates and angles. Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage.  The employer does not ensure that pump jack scaffold brackets are fabricated from metal plates and angles. This violation was observed on or about June 12, 2012, on the exterior of a newly built house where employees were exposed to an approximately 14 foot fall hazard while performing siding work on pump jack scaffolding that was attached at the top with fabricated wood plates.
