29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. On or about June 12, 2012 - at the above addressed jobsite, an employee working in a SKYJACK SJ66T telescopic boom aerial lift was not wearing a body belt or full body harness, thereby exposing the employee to the hazards associated with a fall of approximately eight (8) feet eight (8) inches. THE J.S.L. MASONRY RESTORATION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.453(b)(2)(v), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311942973, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 12, 2008 WITH RESPECT TO A WORKPLACE LOCATED AT 4432 N. TROY, CHICAGO, IL 60473. THE J.S.L. MASONRY RESTORATION, INC.
