Combustable items such as gear oil, rags, and plastic feed bags were stored inside the wooden boxes which housed the switches and breakers for the receiving pit area.

29 CFR 1910.305(g)(2)(ii): Hard-service cord and junior hard-service cord No. 14 and larger were repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced: a) On or about May 4, 2012 and at times prior, Showalter Inc. did not ensure that employees were protected from electrical hazards. The tape used on the outside of the splice was not rated for use in wet and damp locations. Wrapping it around the cord did not bring the outer insulation of the cord to manufacturers specifications.
