Employees were improperly disposing of combustible waste material (waste rag/cloths) in a container without a lid, on or about June 12, 2012.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(b)(2)(i):  All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:         a.  3807 Washington Road, Martinez, Georgia (Shop Area) - Employees were exposed to electrical shock hazard.  The 110V wall electrical receptacle outlet box which was missing the front face cover.      Employees used the outlet to plug in their tools/equipment when needed. The receptacle outlet was energized, on or about June 12, 2012.    No abatement certification or documentation is required for this item.
