29 CFR 1910.22(d)(2): Load(s) greater than that approved by the building official were placed or permitted to be placed on a floor, roof or other structure: Facility: The employer did not ensure through visual observation, appropriate instruction and other means that the structures, such as (but not limited to) storage racks have enough loading capacity prior to permitting employees to have access onto and work/walk on the storage racks. On or about 14 May 2012 the above storage racks were displaced and collapsed while an employee was walking nad working on the storage racks performing change of configuration tasks. The employee sustained severe injuries to the head from the incident.
