OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees    On or about June 12, 2012, three employees were exposed to fall hazards while working in an elevated work platform, 12 feet high, on the forks of an Allis-Chalmers Model #706 material handling tractor.  The platform was 16 feet wide.  The width of the platform was greater than 10 inches past the load bearing tires on each side, which is the maximum width for a stable platform dimension.      Among other methods, feasible abatement methods include:    1.	Use a work platform that does not extend more than 10 inches on each side past the load bearing tires.  2.	Use alternative means to elevate employees to working levels.

29 CFR 1926.602(c)(1)(viii)(A):     The safety platform was not firmly secured to the lifting carriage and/or forks.        Employees working in the sixteen feet wide work platform, on the forks of the Allis-Chalmers Model #706 rough terrain forklift, 12 feet high, which did not have any positive connections between the platform and the tractor forks.  The platform was kept on the forks by the tilting of the forks, which were slid through the channels of the platform.
