29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.      At the job site located at 51 Eager Road, Webster Groves, on 5 June 2012, the employer did not ensure a proper access was provided for employees to access the ladder jack scaffold.  As a result the employee had to climb over the pick board to access the working surface.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

At the job site located at 51 Eager Road, Webster Groves, on 5 June 2012, the employer did not ensure the ladder jack scaffold was placed far enough away from the energized power line or that the power line had been deenergized or covered and insulated to prevent accidental contact.  As a result the employee used the scaffold while exposed to the energized electrical power line.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
