29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    The employer does not fully plank each platform on all working levels of scaffold. This violation was observed on or about June 12, 2012 at the construction site where employees were exposed to a 15 foot fall hazard while working from a scaffold that was not fully planked.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    The employer does not protect employees, from falls when working on scaffolds more than 10 feet above a lower level. This violation was observed on or about June 12, 2012 at the construction site where employees were exposed to a 15 foot fall hazard while working from a scaffold without a fall protection system.

29 CFR 1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.    The employer does not ensure that every pole scaffold has installed diagonal bracing in both directions across the entire outside face of the scaffold. This violation was observed on or about June 12, 2012 at the construction site where employees were exposed to a 15 foot fall hazard while working from a scaffold without diagonal bracing.
