29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    Regulator Department - On or about June 05, 2012 employee operated a single stitch United sewing machine without a needle guard exposing the employee to punctures on the hand and fingers.

29 CFR 1910.219(d)(1):     Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.      Regulating Department - On or about June 05, 2012 employee's right middle finger was amputated while working near a blower. The motor pully was not guarded.

29 CFR 1910.219(e)(4):     Vertical belts. Vertical belts running over a lower pulley more than seven (7) feet above floor or platform shall be guarded at the bottom in the same manner as horizontal overhead belts, if conditions are as stated in paragraphs (e)(2)(ii) (a) and (c) of this section.      Regulating Department - On or about June 05, 2012 employee's right middle finger was amputated while working near a blower. The motor had 2-1/2 inch vertical drive belts that were not guarded.

29 CFR 1910.303(f)(2):     Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.    Regulating Department - On or about June 05, 2012 employee uses an 200 amp electrical panel and the breakers are not marked to indicate their purpose.

29 CFR 1910.1030(g)(2)(i):     The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.    Jobsite - On or about June 05, 2012 the employer did not provide Bloodborne Pathogen training to employees acting as the first responders to an amputation.
