1926.451(c)(1)(i): Crossbraces wer not installed at all locations where horizontal members support both inner and outer legs. On or about April 24,2012, at the worksite located at 428 W. George Nigh Express, McAlester, OK, two employees were working on a scaffold, two sections high. that did not have the crossbracing at both the front and the back of every scaffold sectiion. Employees were exposed a fall of over 12 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about April 27. 2012, at the work site located at 428 W. George Nigh Express, two employees were working on a scaffold at least 10 feet above the ground without any fall protection exposing employees to fall hazards.
