29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a/  At the job site, exposed employees were laying felt paper down upon the northeast corner of the roof.  The overhead power line running from the power pole along the street to the home was located within two feet of the northeast corner of the roof.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    a/  At the job site, exposed employees were removing pre-existing roofing material and installing felt paper without conventional fall protection in place.  The slope of the roof was measured at approximately 5:12.  The ground to eave height was approximately nine feet.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    extended beyond the rake approximately one foot.  The employees were exposed to a fall distance of approximately 15 feeta/  At the job site, the side rails belonging to a portable extension ladder positioned at the west side of the roof .
