29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards.    a) At the Gun Club maintenance shop, in front of the floor mounted bench grinder there was a one half inch diameter bolt protruding from the floor three (3) inches. The protruding bolt constitutes a trip and fall hazard.

29 CFR 1910.215(d)(1): Immediately before mounting, all wheels were not closely inspected and sounded by the user (ring test) to make sure they have not been damaged in transit, storage, or otherwise. The spindle speed of the machine was not checked before mounting of the wheel to be certain that it did not exceed the maximum operating speed marked on the wheel. Wheels should be tapped gently with a light nonmetallic implement, such as the handle of a screwdriver for light wheels, or a wooden mallet for heavier wheels. If they sound cracked (dead), they shall not be used.     a) At the maintenance shop, a Craftsman 1/2 horse power bench grinder mounted on a shop made metal stand, did not have a ring test performed and spindle speed of the machine was not checked before mounting the wheel.
