29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: The employer does not ensure that a guardrail system is installed on all working levels of scaffolding to prevent employee injury. This violation was observed on or about June 11, 2012, when an employee was exposed to a fall hazard while working on a mobile scaffold at a height of twelve feet without a guardrail system in place.
