29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. Station 9+107 - A stairway, ladder or ramp was not located in the 7 foot deep trench. K&K Excavation was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection number 112527262, Citation 1 Item 3, and was affirmed as a final order on 8/23/2010, with respect to a workplace located at Brunswick, Maine.

29 CFR 1926.651(j)(1): Adequate protection shall be provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. Such protection shall consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection. Station 9+107 - Loose rocks were not removed from the face of the excavation.
