On or before June 12,2012, during roofing activities, the employer did not provide programs that included frequent and regular inspections of the job site to identify and eliminate the hazards, such as fall protection, and other hazards associated with the work being preformed at the worksite. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, IDENTITY OF PREVIOUSLY CITED STANDARD 29 CFR .20 (b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 101656, CITATION NUMBER 01, ITEM NUMBER 01, AND WAS AFFIRMED AS A FINAL ORDER ON 03/19/2012, WITH RESPECT TO A WORKPLACE LOCATED AT LISBON, OHIO.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On June 12, 2012, during roofing activities, the employer allowed employees to work on a roof that was approximately 25 feet high with a 5/12 pitch and did not ensure that the employees were protected from falls.
