29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    Mezzanine:  On or about 6/12/2012, and at times prior thereto, the following were found to be inadequate:    1.  The top rail measured 36 inches; 42 inches is required  2.  There was no mid-rail; mid-rail should be placed approximately half-way between the top rail and floor.  3.  The toe board measured 1.5 inches from floor; 4 inches is required

29 CFR 1910.23(e)(3)(iv): The anchoring of posts and framing of members for railings of all types shall be of such construction that the completed structure shall be capable of withstanding a load of at least 200 pounds applied in any direction at any point on the top rail.    Mezzanine: On or about 6/12/2012, and at times prior thereto, the top rail was not mounted securely.

JIP-15MF, SN. 5010097 did not have a guard at the point of operation.    (b) Power Production - On or about 6/12/12, and at times prior thereto, a Sears 15" drill press, SN# 9710800106, did not have a guard at the point of operation.

Power Producition - On or about 6/12/12, and at times prior thereto, the interior cover of the circuit breaker (120 V) for the bay lights/ice maker had numerous openings thereby exposing live electrical parts.
