29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: In the following instances, employees on a steep roof with unprotected sides and edges 6' or more above the ground were not protected from falling: a) Entry way - An employee working on the roof, above the entry way approximately 12' from the ground, was not provided with any form of fall protection. b) Main Roof - The catch platform guardrail system, missing a section of rails, provided inadequate fall protection while an employee worked on the roof approximately 12' from the ground.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. In the following instances, wall openings exceeded 6' or more above the lower levels and the inside bottom edge of the wall opening was less than 39": a) The east wall - The bottom of the door opening approximately 38 1/2" wide x 81" high was approximately 7'4" to the ground below; b) The south wall - The two window openings approximately 32" wide x 48" high were approximately 13' to the ground below. The inside bottom edge of both wall openings were 35" to the floor; and c) The west wall - The two window openings approximately 32" wide x 48" high were approximately 88" and 11' to the ground. The inside bottom edge of both wall openings were 35" to the floor.
