29 CFR 1926.451(f)(3): The competent person did not inspect Scaffolds and scaffold components for visible defects before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about June 07, 2012, the employer failed to ensure that scaffold components were inspected for visible defects before each work shift. Scaffold planks were found to have stress cracks. Employees were standing on the scaffold boards at a height of approximately 30 feet.
