29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.    The employer does not provide a means of egress from the trench excavation.  This violation was observed on or about June 12, 2012, at the excavation site where employees were exposed to falls when entering and exiting an excavation without a means of egress.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    The employer does not extend the ladder 3 feet above the upper landing surface.  This violation was observed on or about June 12, 2012, at the excavation site where employees were exposed to a fall hazard when climbing a ladder that did not extend three feet past the ground level surface of the excavation.

29 CFR 1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    The employer does not protect employees from excavated or other materials that could fall or roll into excavations.  This violation was observed on or about June 12, 2012, at the excavation site where employees were exposed to a struck-by and crushed-by hazard when working in an excavation that had the spoil at the edge of the excavation.

The employer does not protect employees from cave-ins when working in excavations.  This violation was observed on or about June 12, 2012, at the excavations site where employees were exposed to a cave-in hazard when working in an excavation that was 12 feet deep with no protective system.
