The employer is failing to provide employees with adequate training on the recognition of potential hazards on construction sites thus exposing them to potential struck by, caught between and amputation hazards.  The most recent example of this was found at the job site located at 900 G Street, Geneva, NE 68361 where the employer has employees using improper procedures to change the bucket off of a Caterpillar Model 303C mini excavator resulting in the bucket slipping from the hooks pinning his hand and amputating a portion of his ring finger on his left hand.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    The employer failed to annotate and maintain the companys injury and illness logs in accordance with this standard.  The most recent example of this was found at the companys headquarters located at 248 Fairhaven Dr.  Gretna, NE 68028 where the employer had an employee using improper procedures to change the bucket off of a Caterpillar Model 303C mini excavator resulting in the bucket slipping from the hooks pinning his hand and amputating a portion of his ring finger on his left hand.  The employer had not annotated this injury on the OSHA 300 or equivalent form 66 days later.
