29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 14, 2012, an employee working on the second level of a scaffold did not have head protection to protect him from possible head injury from falling objects such as plastering equipment and a five-gallon bucket, which contained plastering mud placed above on the upper level. The employee was exposed to struck-by hazards.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:    On or about June 14, 2012, an employee working on the first level of a two-tier scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards. 29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:    On or about June 14, 2012, the employer did not ensure the scaffold base plate was level, sound, rigid, and capable of supporting the loaded scaffold without displacement. The base plate on the far left and backside of the scaffold was resting only half way on solid cement. The other half was sticking out without a foundation below, making the scaffold unstable. Employees were exposed to a fall hazard of 13-feet to the ground.
