29 CFR 1910.106(g)(3)(iv)(d):     The dispensing units, except those attached to containers, were not mounted either on a concrete island or protected against collision damage by suitable means:    At this facility on the outside northeast corner of the back shop, the employer provides a 500 gallon diesel tank to fuel the powered industrial truck, cranes, etc., in which employees operate and the tank was not guarded from being struck-by vehicles and/or machinery.

29 CFR 1910.157(c)(1):     Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    At this facility, employees are welding during the fabrication of steel tanks for use in the oil industry and fire extinguishers are not mounted and the location is not identified such as but not limited to:    (1) The Amerex Dry Chem Fire Extinguisher at the NE side of the building sitting on top of a work table; and    (2)  The Badger Multi-Purpose Dry Chem Fire Extinguisher sitting on the floor of NW side of the building adjacent to the doorway.

29 CFR 1910.157(e)(3):     Portable fire extinguishers were not subjected to an annual maintenance check:    At this facility, employees perform welding operations on a daily bais to fabricate steel tanks and the fire extinguishers made availabe by the employer had not had an annual inspection performed such as but not limited to:    1.  The Badger Multi-Purpose Dry Chem, 30 pound, serial no. YY19983, sitting by the doorway at the northeast side of the building.    2.  The Sentry, 3 pound, serial no. X494108, mounted on the powered industrial fork truck.
