29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    a) At the Utilities Maintenance Shop, lock was not available to lock out energy isolating device at inspectors test valve.     b) At the Wellhead house, building 563 lock was not available to lock out energy isolating device at electrical panel providing service to the wellhead.

29 CFR 1910.147(c)(5)(ii)(B): Lockout and tagout devices were not standardized within the facility in at least one of the following criteria: Color; shape; or size; and additionally, in the case of tagout devices, print and format shall be standardized.    a) At the Utilities Shop, Tagout devices located in Lock Out Tagout kit were not standarized as to color and size, print and format. Maintenance tags with black on white background, White Hazard Notification - Identification tags with black lettering, Out of Order tags with white lettering on black back ground, Do Not Start tags with white lettering on red circle back ground. No tags had "DANGER, Do Not Start" or similar warning printed on the tag.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) were not trained on the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolatation.:    a) At the well head building employees had not been trained on the specific methods and energy isolating devices to lock out the electrical panel for the well pump.     b) At the Utilities maintenance shop, employees had not been trained on the specific methods and energy isolating devices for the inspectors test valve.
