29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about June 14, 2012, for the employees installing shingles on a roof which had a 6x12 pitch roof and exposed to a potential fall hazard of approximately 20 feet, located at 242 Tenth Street West, Dickinson, North Dakota.
