29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) located at 650 5th Avenue, New York, NY 10019 On or about 6/14/2012 a) Employees were removing wood panels from a barrier wall from a tube & coupler scaffold and worked from a platform that was not planked. An employee fell approximately 16 ft. while standing on the scaffold frames.
