29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: At the job site located at 800 Market Street, St. Louis MO, on 15 June 2012 the employer did not ensure the employee using the JLG aerial lift, model 1250AJP, Serial number 0300119725, wore a body harness to prevent falling from the lift. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
