OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about June 13, 2012, at the worksite located at 100 Crestwood Drive in Beebe, Arkansas, north side of Building 900, filler blocks were not inserted between the rungs on the wooden job-made ladder being used by employees to access and descend the 2nd level balcony area. Employees were exposed up to an approximately 8 ft. fall from ladder to ground below.
