On or about June 11, 2012, the forks of the forklift used to support a scaffold platform was not attached to the forks, exposing the employee to a 16 feet fall hazard. 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a. On or about June 11, 2012, the employee working from an elevated work platform was not protected from falling 16 feet to the ground below by the use of a guardrail system, safety net system or a personal fall arrest system.    Abatement Certification and Documentation is not required
