29 CFR 1926.20(b)(1):     The employer did not initiate and maintain such programs as may be necessary to comply with this part:    Employees conducting roofing operations on a flat pitch roof which were over 6 feet from a lower level were exposed to fall hazards, and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.

29 CFR 1926.20(b)(2):     The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    The employers designated competent person on the site failed to identify existing and predictable hazards and/or take the necessary measures to alleviate the hazard(s) regarding work from a low slope roof which was approximately 18 feet from a lower level.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Employees were exposed to a fall hazard of approximately 18 feet to the ground below while performing roofing work on the top of approximately 3 in 12 pitch roof.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer's training program did not enable each employee to recognize the hazards of falling and was insufficient to train each employee in procedures to be followed in order to minimize fall hazards while employees worked on an 18 foot high roof which had an approximate 3 in 12 pitch.
