29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Laundry:  The specific lockout procedure provided for gas-fired dryers did not include natural gas as an energy source.  Also, dryers were not locked out during lint removal operations that required that the front control panel be removed for access.

29 CFR 1910.213(b)(3):  Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure:    Maintenance Shop:  A Ryobi nine inch vertical band saw was not provided with a drop out type starter or equivalent to prevent the machine from restarting in the event of a power outage with subsequent restoration of power while the machine had been running.

29 CFR 1910.304(g)(5):  The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    Dietary:  Continuity could not be found between the machine frame of the Hobart Model A-200 mixer and the ground prong on the plug.  The grounding conductor provided in the power cord was found to be loose at the connection to the machine frame.

29 CFR 1910.1030(c)(1)(ii)(B):  The Exposure Control Plan did not contain the schedule and method of implementation for paragraphs (d) Methods of Compliance of this standard:    Company:  The Exposure Control Plan had not been updated to reflect the current blood spill clean-up procedures in effect in the facility in that:    1.  The "Blood Spills" portion of the Exposure Control Plan required the use of a diluted bleach solution instead of blood spill kits.    2.  Housekeeping staff was listed as the secondary group responsible for cleaning blood spills.  At the time of the inspection, housekeeping staff was employed by an outside contractor was no longer responsible to clean blood spills.
