29 CFR 1910.147(c)(1): The employer did not establish a program consisting of a control procedure, employee training, and periodic inspections to ensure that be employee performed any servicing or maintenance on a machine or equipment where unexpected energizing, start up or release of stored energy could occur and cause machine or equipment would be isolated, and rendered inoperative in accordance with CFR 1910.147(c)(4): a) The employer did not establish, utilize an energy control procedure (lockout/tag-out program), and provide training on the energy control procedure to ensure that all required elements of the procedure were understood by employees, and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees performing servicing or maintenance on the compactor presses.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Pressing Department - On or about June 11, 2012, the point of operation was not adequately guarded on the Dorst Compacting Press, model #TPA50/2, serial #108 490/1, to prevent employees hands from entering the die area. b) Pressing Department - The employer did not utilize fasteners not readily removable by operator on the compactror presses #1 through #14.
