29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) CH-Line, Bending - Operators of the Trumpf TrumaBend V85 press brake, machine number 085, did set up activities, for example, die-setting on the press brake, which exposed the operator to caught-in or struck-by hazards, etc., and the operators did not utilize lockout/tagout when doing the set up activities, on or about June 13, 2012.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii):     Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:  (a) CH-Line, Bending - Employees used a foot pedal to operate the Trumpf TrumaBend V85 press brake, machine number 085.  Employees bent some work pieces which required the employees to get their hands to within 4 inches of the point of operation, and the press operator did not utilize any point of operation guarding for this work, on or about June 13, 2012.  (b) CH-Line, Bending - Employees bent some large pieces on the Trumpf TrumaBend V85 press brake, machine number 085, that required the press operator and a helper to hold.  The press had one foot pedal, which the press operator used to cycle the press, but the helper had no foot pedal and no other protection from the point of operation for this work, on or about June 13, 2012.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.

29 CFR 1910.253(b)(4)(i):     Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; or in an acetylene generator compartment:  (a) Outdoors Compressed Gas Cylinder Storage Area - The employer had cylinders of oxygen stored next to cylinders of acetylene and methane, on or about June 13, 2012.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and shall state that employees and their representatives were informed of this abatement.
