29 CFR 1926.416(a)(3):   Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:    a.  3200 Augusta Tech Drive, Augusta, GA (Vocational Magnet High School) - The employer failed to ensure employees from working on (tapping wires) and/or being in close proximity to energized equipment. The foreman did not consult the H wing and J wing lighting plans and/or diagram which were in the possession of the foreman, before allowing employees to enter or to work an area that had energized 277V circuit (circuit 1), on or about 06/13/12.

29 CFR 1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that:    a.     8141-D Technology Drive, Covington, GA (Office) -The employer did not maintain the injuries and illnesses logs (OSHA-300) where work related incidents occurred for the annual year of 2012.  An employee was injured, cut left arm,  while working using wire strippers when stripping the electrical wires on March 27, 2012.  The employee needed stitches and was given lost and/or restriction workdays.  The incident was not recorded on the injuries and illnesses logs, on or about 06/13/12.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
