29 CFR 1926.403(d)(1): Mounting. Electric equipment shall be firmly secured to the surface on which it is mounted. Wooden plugs driven into holes in masonry, concrete, plaster, or similar materials shall not be used. a. On the first floor where energized electrical panels were located, The electrical panels were not securely mounted to the wall but were hung from wires on the ceiling thereby exposing employees to a contact hazard. b. On the second floor where energized eletrical panels, were located, the electrical panels were not securely mounted to the wall but were hung from wires on the ceiling thereby exposing employees to a contact hazard.

29 CFR 1926.405(g)(2)(v): Cords passing through holes. Flexible cords and cables shall be protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures. a. In the teledata room 1/B where the temporary electrical was connected, the tempoary electrical cord coming out of the electrical panel was pulled from the panel and not provided with bushings thereby exposing employees to a contact hazard.

29 CFR 1926.405(j)(1)(i): Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts. a. On the fourth foor, Area C the temporary lighting was installed 20 inches from the floor where it was connected to the electrical outlet thereby exposing employees to a contact hazard.
