29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) Jobsite: 877 New Castle Road, Slippery Rock, Pa 16057; On or before June 15, 2012, employees were working unprotected and exposed to fall hazards of approximately 18 feet 4 inches to ground level while on a 4/12 pitched roof. The employees were working to retrieve and install new shingles.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Jobsite: 877 New Castle Road, Slippery Rock, Pa 16057; On or before June 15, 2012, employees did not receive training from their employer on the hazards associated with falling while applying roofing materials on a low-sloped roof and what fall protection methods shall be used.

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) Jobsite: 877 New Castle Road, Slippery Rock, Pa 16057; On or before June 15, 2012, employees were exposed to fall hazards of approximately 18 feet 4 inches while using a fiberglass extension ladder to gain access and egress from a roof in that the end of the ladder rested against the eave and did not extend above the roofline at all.
