Materials and equipment were placed, either permanently or temporarily, within the exit route: a) On May 21, 2012, on the northwest side of the warehouse, the exit route to the fire exit door was blocked by furniture and equipment, creating a hazard for employees to exit in the event of an emergency. b) On May 21, 2012, on the northeast side of the warehouse, the exit route to the fire exit door was blocked by furniture and equipment, creating a hazard for employees to exit in the event of an emergency.
