29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: a) Plainfield Police Station, 210 Norwich Rd., Plainfield, CT. - On or about 6-15-12 - Employer was using a home made platform attached to a Lull lift Model 844C-42 to hoist employees and material for roofing operatioins. The platform extended 24 inches past each wheel of the lift. The lift is 8 feet wide and the platform was 12 feet wide. The manufacturer allows the platform to extend only 10 inches beyond the wheels. The platform was not approved for use by the manufacturer, Lull International, Inc.

29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) Plainfield Police Station, 210 Norwich Rd., Plainfield, CT. - On or about 6-15-12 - Employer was using a home made platform attached to a Lull 844C-42 lift to lift employees and material for roofing operations. The platform was 12 feet wide and the lift was 8 feet wide, 24 inches over each wheel. The lift was manufactured as per ANSI B56.6 standards which allows load capacity at 10 inches over the width of each wheel.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. a) Plainfield Police Station, 210 Nowich Rd., Plainfield, CT. - On or about 6-15-12 - Employer was using a home made work platform to hoist employees and material to install asphalt shingle roof systems. The platform which was mounted on the forks of a Lull 844C-42 which prohibited the use of the machine to lift personnel.
