Collis Roofing, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in the following inspections:         1.  Number 315570697 Citation 01 Item 001 and was affirmed as a Final Order on May 17, 2011, with respect to the              workplace located at 136 N. 4th St., Lake Mary, Florida 32746.           2. Number 314225913 Citation 01, Item 001 and was affirmed as a Final Order on April 6, 2010, with respect to the           workplace located at 6082 Saint Julian Dr., Sanford, Florida 32771.
