29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) North Side of the Site: A laborer was observed working below two workers without head protection. The employees working above were using hand tools while installing a false store front. This condition exposed the employee to a potential head injury from being struck by falling objects.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. (a) Southeast Corner of the Site: Employees using electrical power tools to cut plywood were not protected by a ground fault circuit interrupter receptacle. Employees were exposed to a potential electrical shock hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Northwest Corner of the Site - A carpenter was observed without fall protection while on top of a two staged container. The employee was exposed to a fall from elevation of approximately 16 feet to the pavement below. b) North Side of the Site - A carpenter foreman was observed without fall protection while on top of a single staged container. The employee was exposed to a fall from elevation of approximately 8 feet to the pavement below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Southwest Corner of the Site - An extension ladder that was placed on top of a container, was used by employees to gain access to another stacked container. The ladder did not extend at least 3 feet past the landing level of the stacked container (conex box). Employees were exposed to a fall from elevation of approximately 8 feet to the container below.

The step ladders were used by employees to gain access to upper levels. Employees were exposed to a fall from elevation to the next lower level.
