29 CFR 1910.106(d)(7)(i)(b):  The employer did not ensure that at least one portable fire extinguisher having a rating of not less than 12-B was located not less than 10 feet, nor more than 25 feet, from any Class I or Class II liquid storage area located outside of a storage room but inside a building.    (a)  East side of building: The fire extinguisher located in the east side of the building was not charged to the correct pressure as indicated by the gage.  The extinguisher was not rated to at least 12-B.  Class I liquid gasoline for the chain saw was stored in this part of the building on or about June 18, 2012.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(g)(2)(i):  Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures:    (a)  Band Saw Station: The cord providing power to the band saw was spliced using wire nuts, and electrical tape.  This method of splicing did not retain the original insulation factor of the wire manufacturer.  This work station was located outside in a temporary structure which was exposed to rain and other forms of precipitation.  The spliced wire was located within inches of a pile of combustible saw dust, on or about June 18, 2012.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(1)(i):  Openings through which conductors enter boxes, cabinets, or fittings were not effectively closed.    (a)  Skid Tear-Down Band Saw - The strain relief fitting for the cord to the band saw was not secured in place and did not effectively close the opening into the machine's electrical switch housing. Entry of foreign materials including but not limited to combustible saw dust was not prevented on or about June 18, 2012.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(a)(2)(v)(C): Branch-circuit conductors were laid on the floor.    (a)  Band Saw Station: The Romex branch circuit conductor providing power to the band saw was run along the ground from the circuit breaker panel box to the band saw.  There was damage to the outside insulation of the Romex cable, on or about 18 June 2012.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
