29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    A) At 12350 Mercy Blvd. Savannah, on or about June 15, 2012, employee was observed climbing up an extension ladder under roofers, not wearing personal protective equipment, exposing employee to head injury from impact or falling objects.      ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1926.307(d)(1):     "Guarding." Pulleys, any parts of which are 7 feet (2.128 m) or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.    a) At 12350 Mercy Blvd. Savannah, on or about June 15, 2012, employees operating compressor with unguarded pulleys were exposed to amputation and laceration hazards.      ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1926.307(e)(1)(i):     Where both runs of horizontal belts are 7 feet (2.128 m) or less from the floor level, the guard shall extend to at least 15 inches (38.1 cm) above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches (106.68 cm) or less from the floor, the belt shall be fully enclosed.    a) Hazards-Operation/Condition: At 12350 Mercy Blvd. Savannah, on or about June 15, 2012, employees operating compressor with unguarded belt were exposed to amputation and laceration hazards.       ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    A)     At 12350 Mercy Blvd, on or about June 15, 2012, employees were installing shingles on a roof that had a ground to eave height of 18 feet and a roof pitch of 2:12 pitch.  The employees were not fall protected by conventional fall protection or alternative means such as slide guards or roof monitor, exposing employees to fall hazards from about 18 feet to the lower level ground.

29 CFR 1926.1053(b)(4):     Ladders shall be used only for the purpose for which they were designed.    A)  At 12350 Mercy Blvd, on or about June 15, 2012, employees used a step ladder leaned against an extension ladder to get on and off the roof.  The stepladder was designed to be used in the open or A position as a self-supporting ladder, per the manufacturers recommendation.      B)   At 12350 Mercy Blvd, on or about June 15, 2012, employees used an extension ladder that the non slip feet on the extension ladder were up, exposing employees to fall hazard.

29 CFR 1926.1053(b)(22):     An employee shall not carry any object or load that could cause the employee to lose balance and fall.   [55 FR 47689, Nov. 14, 1990; 56 FR 2585, Jan. 23, 1991; 56 FR 41794, Aug. 23, 1991]    a)  Employees were exposed to fall hazards.  An employee was observed decending an extension ladder while carrying a wheelbarrow on his head.  Another employee was ascending the ladder while carrying a 5 gallon bucket with materials in it.
