29 CFR 1926.453(b)(2)(v):  A body belt was not worn and a lanyard was attached to the boom or basket when working from an aerial lift:    Jobsite:  The employee who was working from aerial lift basket was not wearing a full body harness with the lanyard attached to the basket or boom.    Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. 29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Jobsite:  The ladder that was used to gain access to the roof did not have its side rails extended 3 feet above the landing surface.

29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    Jobsite:  The employees, installing roof sheathing, were not protected from fall hazards by the use guardrails, safety nets or personal fall arrest systems.    Drywall Unlimited Corporation was previously cited for a violation of an equivalent Occupational Safety and Health standard  29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 311784045, citation number 1, item number 2 and was affirmed as a final order on 8 March 2010, with respect to a workplace located at 83 West Main Street, Plantsville CT  06479.    Drywall Unlimited Corporation was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 311796544, citation number 1, item number 1 and was affirmed as a final order on 9 March 2010, with respect to a workplace located at 1596-1610 West Street Southington, CT  06489.
