Location: Warehouse/Outdoors    Employer did not take forklifts out of service that were in need of repair.     1) Forklift 9 (S/N 7277):    a) Crushed metal screen load apron    b) Windshield wipers inoperable     2) Forklift 5    a) Backup alarm inoperable     3) Forklift 10 (S/N 71267):    a) Horn inoperable    b) Windshield wipers inoperable      c) Backrest is missing bolt and was welded.  This was not a manufacturer's  authorized repair.    		  4) Forklift 15 (S/N 71036):    a) Horn inoperable    b) One headlight inoperable    c) Back strobe light inoperable    d) Windshield wipers inoperable    e) One fork is bent/twisted and is higher than the other, causing vehicle instability during loading/unloading.
