29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.        On or about June 13, 2013, an employee was exposed to a fall hazard while working on a platform that extended more than 18 inches over its support.  No guard rails were installed to prevent the employee from accessing the end of the platform and causing it to tip.

Cross bracing on the lower level of the scaffold was not properly secured.  Employees were exposed to the scaffold collapsing.

On or about June 13, 2012, employees worked from a scaffold erected with an unguarded walkway exposing employees to a fall of approximately 13 feet to the ground.
