29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a) At the building tower, on or about June 13, 2012, employees who were working while standing on supported scaffold platforms that were set-up on the main roof and tower window frames were not using any means of fall protection, exposed to a 17' fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Near the building tower, on or about June 13, 2012, employees who were working while standing on the 6:12 sloped roof were not provided with any means of fall protection, exposed to a twelve-foot fall hazard. A To Z Home Improvements, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 313877953, citation number 1, item number 1, and was affirmed as a final order on October 14, 2010, with respect to a workplace located at Success Drive - Chipola College, Marianna, FL 32444.
