29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about 6/19/12, in Area C, on Construction site, 77 GoodellSt., Buffalo, NY: Three employees installing a "Green Roof" on the flat roof of the building were exposed to a potential fall of approx. 17 ft. 6 in. from the south, east, & west edges of the roof. All of the employees were wearing full body harnesses. An engineered steel cable had been installed around the perimeter of the roof, 7 ft. in from the edge. Each employee was provided with a 9 ft. retractable lifeline to attach to the cable and to his/her harness. Employees were not always tied off within 15 ft. of the roof edge and had to walk towards the roof edge in order to clip the retractable lanyard to their harness. No warning lines had been erected and there was no monitor. NO ABATEMENT CERTIFICATION REQUIRED
