29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: a.) Fabrication shop: On or about 06/19/12, the yellow extension cord hanging from the ceiling, plugged into an overhead mounted receptacle and used with a portable heat gun was tested to have reverse polarity in that the neutral and hot wires were reversed.

29 CFR 1915.504(a): Written fire watch policy. The employer must create and keep current a written policy that specifies the following requirements for employees performing fire watch in the workplace: a.) Earl Beach Panels & Control: On or about 06/19/12, a written fire watch policy was not created prior to employees performing fire watch duties onboard the vessel Cape May. Abatement Note: The following must be included in the written fire watch policy: 1. The training employees must be given (1915.508(c) contains detailed fire watch training requirements); 2. The duties employees are to perform; 3. The equipment employees must be given; and 4. The personal protective equipment (PPE) that must be made available and worn as required by 29 CFR Part 1915, Subpart I.
