Location: 245 W. 25th St., New York, NY. a) Employer failed to ensure that the an extension ladder utilized by employees was properly fixed. The extension ladder was made of wood and components from an A frame ladder; on or about 6/13/12.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. Location: 245 W. 25th St., New York, NY. a) Employer failed to ensure that the an extension ladder utilized by employees was properly fixed. The extension ladder was made of wood and components from an A frame ladder. Ladder training is required; on or about 6/13/12.
