29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it is safe and defective rigging equipment was not removed from service: For work conducted at an excavation site on or about June 15, 2012, sling inspections were not being performed before each day's use. The sling with visible defects was used to lift, carry, and lower 3,000lb precast irrigation joints into trench exposing employees to the hazard of being crushed by the falling irrigation joints.
