Construction site: On or about June 19, 2012, three employees were observed working at the edge of the roof without any form of fall protection. The employees were exposed to fall hazards of approximately 11-feet, 4-inches to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Construction site: On or about June 19, 2012, two employees were observed using a 10-foot A-frame Ladder to egress the roof where the ladder side rails did not extend at least 3 feet above the upper landing surface. The employees were exposed to fall hazard 11-feet, 4-inches from the ground.
